• Please bring any vaccination records you have and photo identification, such as a passport, driver’s license, or any other official form of photo ID. You will have a physical exam, and our physician will evaluate what vaccines and tests you will need.
  • Depending on your age, we will give you order forms for lab tests for gonorrhea, syphilis & TB. Lab results can take up to 5 working days. We will call you when the lab results come back and set up an appointment for the 2nd visit.
  • We will give you needed vaccines or give you orders for you to get them somewhere else if you so desire.
