How do I fill out the I-693 form?

Please do not bring your own I-693 form; we will provide you with the I-693 form when you are at the clinic. If you need assistance filling out the form, we will be happy to help.

Form Filling Instructions

One form per applicant is required. Fill out the form only in black ink. Fill out part 1: Information about you on the first page of the form. Fill out part 2: Applicant statement and Contact information on the second page of the form. Each page's top line has boxes asking for your family name, first name, and A-number. Please fill these boxes on all pages. If you do not have an A-number, leave it blank. If you need an interpreter, please check with the staff at the clinic what information you should put in part 3 (interpreter section). Fill out the preparer’s information in part 4 only if needed.

How long does the medical exam process takes?

The medical exam process will require two visits. The average time for the expedited process is 6-7 days, depending upon your vaccination, lab results, and your timely follow-up.

Please bring any vaccination records you have and photo identification, such as a passport, driver's license, or any other official form of photo ID. You will have a physical exam, and our physician will evaluate what vaccines and tests you will need. Depending on your age, we will give you order forms for lab tests for gonorrhea, syphilis & TB. Lab results can take up to 5 working days. We will call you when the lab results come back and set up an appointment for the 2nd visit. We will give you needed vaccines or give you orders for you to get them somewhere else if you so desire.

On your second visit?

Our USCIS civil surgeon will review your I-693 completed package, vaccines, and lab results and see if anything else is required per USCIS guidelines. You will be given a sealed envelope of your exam to be submitted to USCIS or your attorney. A duplicate copy will also be provided to you. Sometimes, if the TB test comes back positive, you will need a chest X-ray, which requires an extra 1-2 days, depending on when you can get the x-ray done. We are here to help & guide you in your medical exam. However, we defer legal questions for you to ask your attorney. Please let us know if you have to meet deadlines.

What vaccinations are required?

Our physician will review and determine which vaccines are still needed. As per USCIS requirements, you need the vaccines listed below. However, if your record shows that you have already received any of these vaccines in a timely manner, you will not need them again. a) Adults: Tdap, MMR, Hepatitis B, and Polio. Varicella (Chicken Pox) is required only if you did not have chickenpox disease in childhood. b) Children: Must be up to date with the vaccination schedule for their age per CDC guidelines for Hepatitis A and B, Influenza, Haemophilus influenza b (Hib), measles, mumps, & rubella (MMR), Meningococcal, Pneumococcal, Pertussis (whooping cough), Polio, Rotavirus, Tetanus & Diphtheria and Varicella (chicken pox). c) Meningococcal B vaccine is required from ages 19-23. d) Hepatitis B vaccine: Effective June 2022, Hepatitis B is required for ages under sixty. e) Polio vaccine: Effective May 2024, Polio vaccine is required for all ages f) Pneumonia Vaccine is required for age over sixty-five. g) Flu vaccine is required annually during flu season for age over six months. h) MMR vaccine is required for anyone born in 1957 or later. i) COVID-19 vaccine booster is no longer required for immigration. Declining the COVID vaccine booster will not affect your immigration process. Our physician will determine any gaps during your medical exam. Even if you do not have any vaccination records, you will be presented with all available options to meet your requirements for your health needs, cost, and expediency for filing the I-693 form. What if I do not have vaccine records?

Vaccines are required by USCIS for communicable diseases. Bring any records you may have. We can take any vaccine records that were administered by anyone, including in a foreign country. The physician will do their best within their discretion and USCIS guidelines based on your medical history and give you choices to either do a test for immunity or give vaccines. You can decide based on your filing timeline, cost, and the health & safety of others.

Are vaccines safe? What if I am pregnant or sick?

Yes, vaccines are safe. You can take another shot even if you have taken it before. Consult your physician if you are pregnant, sick, or allergic to ingredients. You will need a waiver from your physician for USCIS for these conditions. Speak to the physician or civil surgeon during your medical exam for guidance.

What lab tests are required?

1. QuantiFERON (Tuberculosis) for all ages 2. Gonorrhea for ages 18-24. 3. Syphilis for ages 18-44. All your lab tests are done by an outside accredited USCIS-approved lab. As per Immigration Services requirements, these lab tests must be ordered by a civil surgeon and are not accepted through other parties. We will give you Orders/Guidance regarding labs after the Medical Exam. The lab results take up to 5-6 working days to come back.

Lab tests are covered by your private insurance and Medi-Cal. If you do not have insurance, you pay our special price of only $50 extra for your labs.
